Treatment Plan and Payment in One Step: Why Clinics Lose Money Between the Chair and the Desk

The doctor explains the plan, the patient agrees, and then the patient is sent out to reception to "settle the payment". Somewhere in those ten metres, half the commitment evaporates. Clinics that put the plan and the payment on the same screen close more treatment and chase fewer balances.

Every clinic has a version of this: Dr. Ayesha spends fifteen minutes explaining an RCT and crown to a patient, shows the X-ray, answers questions, and the patient says yes. She writes the plan, and the assistant walks the patient to reception. At reception, the plan has to be re-entered or looked up, prices are checked, and the patient says, "I will come back next week to start". Sometimes they do.

The treatment was accepted in the chair and lost at the desk. Closing that gap is one of the quickest revenue wins available to a Pakistani clinic.

Where the money leaks

  • Time between yes and paying. Every minute between agreement and payment is a minute for doubt.
  • Re-entry at the desk. The plan is rewritten for billing, prices are looked up, mistakes creep in, and the patient waits.
  • Vague pricing. If the patient is not shown an itemised total, the number at the desk feels like a surprise.
  • Estimates that never become invoices. A plan saved as a quote sits there forever unless someone converts it.

The one step flow

  1. Build the plan from the service list. Type a treatment, pick it, and its rate fills in. Change the treatment and the price follows.
  2. Save the plan. For clinics that work this way, saving makes the plan active and creates the invoice immediately. No separate conversion step.
  3. Show a summary. Each treatment, the tooth, the rate, discounts and the net payable on one screen.
  4. Collect. Press Collect payment and take the full amount, the first installment or an advance, right there.
  5. Receipt on WhatsApp. The patient leaves with the receipt on their phone and the next appointment booked.
The summary matters more than it looks. A patient who sees "RCT 26, Rs 20,000 · Zirconia crown 26, Rs 35,000 · Net Rs 55,000" before paying rarely argues. A patient told "fifty five thousand" at the desk often does.

Estimates vs active plans

Neither is wrong; they suit different clinics.

Estimate firstActive when saved
Best forPatients who compare quotes, big cosmetic casesPatients who agreed in the chair
InvoiceCreated later, when the patient agreesCreated the moment the plan is saved
PaymentCollected on a later visitCollected immediately
RiskQuotes that never convertMust be explained clearly before saving

Many clinics run both: estimates for cosmetic and implant consultations, active plans for routine and emergency treatment. Good software lets the clinic choose which way it works.

Installments without losing track

Large plans in Pakistan are usually paid in parts. Collecting the first installment at the chair commits the patient; the remaining installments are scheduled against the same invoice, and each payment gets its own receipt showing the balance. Our guide to installment plans covers how to structure them.

One page, not two

The underlying idea is simple: the plan page and the payment page should be the same page. The doctor or assistant sees the plan, the invoice it created, the payments against it and the balance, and can collect without switching screens or handing the patient to someone else. For clinics that want it, the separate finance tab disappears entirely and everything lives on the plan.

What changes when you do this

  • More treatment started on the day it is agreed.
  • Fewer outstanding balances to chase.
  • No re-entry errors between plan and bill.
  • Patients who feel the process was clear and professional.

Pair it with the techniques in increasing treatment plan acceptance and the planning side in treatment planning software.

The bottom line

The yes happens in the chair. Take the payment there too, or at least on the same screen, with an itemised summary in front of the patient. The ten metres to reception are where clinics lose treatment they had already won.

Frequently asked questions

Why do patients agree to a plan and then not pay?

Because agreement and commitment happen in different places. In the chair the patient is engaged with the doctor; by the time they reach the desk they have had time to doubt, check their phone and decide to think about it. Collecting the first payment while the plan is fresh converts agreement into commitment.

Should every plan start as an estimate?

Not necessarily. Estimates suit clinics where patients shop around. Clinics where the patient has already agreed in the chair often prefer a plan to be active and invoiced the moment it is saved, with payment collected straight away.

How can treatment prices be filled automatically?

By picking each treatment from the clinic's own service list as you type. The service carries its rate, so choosing Zirconia crown fills in the price, and changing the treatment updates the price too.

What should the patient see before paying?

A short summary: each treatment, the tooth and the rate, any discounts, and the net payable. Seeing it itemised reassures the patient and prevents the "I thought it was less" conversation.

Does collecting at the chair work with installments?

Yes. The first installment or down payment is collected on the spot and the rest is scheduled, with each payment receipted against the same plan invoice.
