The FDI Tooth Numbering System, Explained (With Every Number)

Why tooth 36 is a lower-left first molar, how the two-digit system works, what 51–85 mean on a child, and how FDI compares with the Universal and Palmer systems you will meet in imported software.

Every dentist learns tooth numbering once and then uses it for forty years without thinking about it — until a new team member charts 26 when they meant 16, a referral comes back describing tooth 14 when the lab meant something else entirely, or an imported software package silently numbers teeth 1 to 32 and nothing matches the notes.

Here is the system in full, plus the two rivals you will bump into.

Two digits, and both mean something

FDI notation (ISO 3950) gives every tooth a two-digit number. The first digit is the quadrant; the second is the position, counting outward from the midline.

Quadrants for permanent teeth run clockwise from the patient's upper right, as you face them:

Position runs 1 to 8 from the midline outwards: 1 central incisor, 2 lateral incisor, 3 canine, 4 first premolar, 5 second premolar, 6 first molar, 7 second molar, 8 third molar (wisdom tooth).

So: 11 upper-right central incisor · 23 upper-left canine · 36 lower-left first molar · 48 lower-right wisdom tooth.

Read them as digits. "Four-eight", not "forty-eight" — the number is a coordinate, not a quantity.

Left and right belong to the patient

The commonest error in charting is not the numbering — it is the mirror. Quadrant 1 is the patient's upper right, which appears on your left as you stand in front of them. A chart drawn from your point of view rather than the patient's flips every entry, and it is a mistake that reaches the extraction chair.

Primary teeth: quadrants 5 to 8

Children get their own quadrant digits, in the same clockwise order, with five teeth per quadrant:

Positions run 1 to 5: central incisor, lateral incisor, canine, first primary molar, second primary molar. Twenty teeth in total.

This is what makes FDI so well suited to a mixed-dentition child, which is most paediatric dentistry: a seven-year-old may have 16 erupted alongside 55 still in place, and both numbers are unambiguous on the same chart. A chart that can only show one dentition at a time forces the clinician to pick, and picking loses information.

The other two systems, so you recognise them

Universal (US)

Permanent teeth numbered 1 to 32 in one continuous sweep — 1 is the upper-right wisdom tooth, 16 the upper-left wisdom tooth, 17 the lower-left wisdom tooth, 32 the lower-right. Primary teeth use letters A to T. No quadrant information is encoded, so the number tells you nothing without a lookup.

Practical warning: much American software defaults to this, and "tooth 14" means the upper-left first premolar in Universal but the upper-right first premolar in FDI. Same words, different tooth, opposite side of the mouth.

Palmer

Quadrant symbols (⌐ ¬ L ˩) with 1–8 inside, primary teeth A–E. Intuitive on paper, awkward in software and databases because the symbols do not type cleanly — which is why it has largely given way to FDI in digital records.

Why this matters beyond the exam

Notation is the shared vocabulary between the dentist, the assistant charting, the lab making the crown, the colleague you refer to, and the software storing it all. Every mismatch is an opportunity for the wrong tooth.

Three practical consequences for a clinic:

  • Pick one system and enforce it across notes, referrals, lab slips and prescriptions.
  • Check what your software uses. If it was built for the American market, it may be storing Universal numbers behind an FDI-looking chart — or vice versa.
  • Chart both dentitions for children. A mixed-dentition patient needs adult and primary teeth on the same screen, not a mode switch that hides half the mouth.

Frequently asked questions

What does tooth 36 mean?

The first digit is the quadrant and the second is the tooth\u2019s position from the midline. 3 is the lower-left quadrant (patient\u2019s left) and 6 is the sixth tooth from the centre — so 36 is the lower-left first permanent molar. Spoken as "three-six", never "thirty-six".

Why do child charts use 51 to 85?

Primary teeth get their own quadrants — 5, 6, 7 and 8 — running clockwise in the same order as the adult quadrants, with five teeth each instead of eight. So 51 is the upper-right primary central incisor and 85 is the lower-right second primary molar. It keeps a mixed-dentition child unambiguous: 16 and 55 are clearly different teeth.

Is FDI the same as the Universal numbering system?

No. Universal (used mainly in the United States) numbers permanent teeth 1 to 32 in a single sequence and letters A to T for primary teeth. FDI uses two digits, is quadrant-based, and is the ISO standard used across most of the world including Pakistan. Software built for the US market frequently defaults to Universal, which is a real source of charting errors here.

Which system should a clinic in Pakistan use?

FDI. It is what dental schools here teach, what colleagues and labs expect on a referral, and what the ISO standard specifies. If your software cannot chart in FDI, that is a genuine problem rather than a preference.
